Without these documents you will not be able to start with us. Have them ready and registration moves quickly.

Home Health Aide Certificate (75hrs) or CNA certificate
Background Screen Level 2 (every 3 years)
OSHA certificate (annual)
HIV/AIDS Certificate (biannual)
Alzheimer Certificate (biannual)
Domestic Violence (biannual)
HIPAA (biannual)
Patient Rights (biannual)
Current physical within previous 6 months stating “free of communicable disease” (annual)
Current PPD (required annually)
Current CPR
Social Security Card
Proof of Residency or Citizenship (Work permit / Alien card, Voter registration or Passport)
Driver License or Florida ID
Auto Insurance
Professional Liability Insurance
2 Reference letters and/or verification of employment contact

All documents must be current prior to registration.
